We round onsite several times a week, coordinate with therapy teams, and keep your primary doctor and family in the loop — all to speed recovery, prevent readmissions, and get you safely home.

What to expect

First physician visit within 24–48 hours of arrival at the postacute facility
Detailed medication reconciliation to avoid duplications or omissions
Regular bedside rounds (at least twice weekly) plus oncall coverage 24/7
Seamless coordination with PT/OT, wound care, and specialty consultants
Early identification of changes in condition to reduce ED transfers
Clear updates sent to your primary care provider and family caregivers
Transition planning for home health, outpatient therapy, and followup visits
If clinically warranted, a warm handoff to our Platinum Continuity & Specialty Clinic for followup care
